To add a Service Record of Discipline, click *Discipline Records* on the left menu.

Search the member by name or select a member already listed.

Click the Misconduct Records tab and then the Create New Entry Button.

### **Discipline Record Fields**

The data fields on a discipline record are intended to capture all the necessary information to facilitate disclosure, including:

* The discipline or correction that was applied

* File Reference numbers: RMS (PRIME), Professional Standard case number, OPCC file number. Substantiation and Expungement dates (Note: the files names below are a BC example, but can be modified per agency.

* Agency - identifies the "holding agency" in cases where the misconduct complaint was during previous service at another agency.

* Allegation - multi-select field documenting the allegations of the complaint

* Mode - draft, published or expunged

* Rank - of Member at time of complaint

* Status - of Member - Active, Inactive, Suspended, Retired

### **Text Fields**

The Text fields on the Discipline Record are intended to make the disclosure process for efficient. The text boxes are used to generate the [Secondary Disclosure](https://secondary-disclosure---professional-standards/) documents submitted to Crown by Professional Standards.

Note: The Relevant and Not Relevant text boxes were for pre-McKee workflow. If your agency is disclosing all records and no longer determining relevance, please use the Summary box
